Multiple Choice Input Form

Does anyone know how to enable my web forms to allow Multiple choice Input? I also want to create a wide range of input fields, say like address showed in Google maps , rating, dropdown, and websites. If not, do you know any plugin would help me out as simple as possible? Thanks a lot!
