How can I create Dependent Custom Option?

I need the extension to realize the possibility that the second drop-down is supplied based on the selection of the first and the value varies depending on the choice of the type of input in the first field

And i want to show a drop-down selection at say 2 is selected, and another value when 3 or 4 is selected.

Please help create one this extension.
