You can create a custom module and call this in footer so it will include this form in all pages including home page as well.

There should be a multiple approach to achieve this.

Let's say you can put this script in the root directory of your magento 1 and on your magento 1 site you can create a link and that link is pointed to this script - so customer can click on that link and redirect to this feedback form link and fill-up the form.

Second approach should be create a custom module which call the PHP scripts into your cms page or you same way you can give link on header or footer and that will accessible on all the page - customer can click on that link and fill up the form for the same.

You can get the help from the above link  i have posted.

You need to create a custom module - where you need to put css and js file in respected directories.

Then you need to create .phtml file where you required to put your html and PHP script code.

You also required to put routers in your config.xml file to get the actual URL of that custom module.
