Need Help. Programmer Bailed on me

I am new and not savoy to technical terminology.  Programmer bailed out on us before working out the kinks on our site.  We need to make one field in the checkout process mandatory.   

We have children's program. Customer is parent but MUST have child's name to trace who payment is being made for.  I think we could do it at the check-out time but need to make Child's Name mandatory to check out.  We are using Pay Pal but they say we can't make it happen through their process.

You can add text field with the name of child and make it mandatory by using this Magento Add attribute to order extension. This extension supports 9 different types of field like date, checkbox, radio button, drop-down etc. You can add fields on both checkout and registration page.
