Magento 1.9.2.0 SUPEE-8788 v2 Hunk #1 FAILED at 91

I revert SUPEE-1533 as sugusted successfully but when I try to patch SUPEE-8788, I get "Hunk #1 FAILED at 91" Please see the log below

I followed the guide on  SUPEE 8788 AND SUPEE 1533 (Incompatible, Hunk error) I put the file in a zip, then upload it to the root directly. Extract the zip, then run the patch as per Magento instructions. that got rid of the swf error

I have still got two errors, the first one is:

Hunk #1 FAILED at 91.

I did not install the SUPEE-8788 v1 before 8788 v2 so I don't know why I am getting this error.

The second error is

Perhaps you used the wrong -p or --strip option?

I have checked the files on my Magento root folder and I have got "app/etc/modules/Mage_All.xml" but the lines only goes up to 401. I have not got any file called "app/locale/en_US/Mage_Media.csv" which I believe the patch is looking for.

I hope this will help me get an answer for my issue.

I seems like with Magento version 1.9.2.0 you have to have SUPEE-1533 installed in order to patch SUPEE-8788 v2. I reinstalled 1533 and now that got rid of the

"Hunk #1 FAILED at 91" patching file app/code/core/Mage/Adminhtml/controllers/DashboardController‌​.php

Now I have only got the last error:

I managed to installed mine by copying a new Mage_Media.csv from a fresh Magento directory into my directory on my server. But now https://www.magereport.com/ can not read my Magento version "Outdated Magento version? unknown" as well as not been able to read that my Magento file is protected "Unprotected development files? unprotected" Also https://magentary.com/magento-security-patch-tester/ reports 7405 not installed.?
