Duplicate URL Rewrite Issues

Since August we have experienced a major issue of thousands of URLs being created for a single product, when I look in the URL rewrite section one bracelet has 15794 url rewrites for a single product. This is causing major issue for us and affecting our SEO. Can anyone help?
