Weird items/billing and shipping address - need help!

So I have been testing my website. if you use a CC payment. (manual) the order goes through however there are weird items added to every order along with very odd shipping/billing addresses. Has anyone ever experienced this before? Everything else is working as normal pretty much. 

Any help is GREATLY appreciated! 

Thoughts? You can see in the image. The item for $9.99 was the one originally added to the cart. The two above them are just random things that I don't have in the catalog.

There is any Shopping Cart Price Rules? Or using any third party?

When using Shopping Cart Price Rules, there is an action: Buy X get Y free (discount amount is Y). You should check Promotions. May cause you problem.

Oddly enough I did see some generic catalog price rules. I removed them but am still getting the same result. I purchased the theme so maybe there is information saved from somewhere else? I appreciate your help mr Khoa! At the bottom it says it notified customer from in 2013.
