Unable to see preview thumbnail or edit controls for images

As admin user, I am unable to see the preview thumbnail or edit controls for images in the admin area, so I cannot upload new images. 

Instead I can see a pixel where the preview should be, but it isn't clickable.

Any ideas would be greatly appreciated!!
