Missing homepage <titles> from sitemap.xml

We have had some problems with the sitemap.xml generation, my developer appears to have solved them.  

But there is missing <title> tags for our homepage and a couple of other pages, see here: https://www.screencast.com/t/zVwDfaCzL

I am not sure why?
