Calculate shippping in a separate page

Hello, 

I am looking for a way to let my customer estimate their shipping cost by filling out a form with the weight and dimensions of their package and see all options available with the prices just like this: https://zenmarket.jp/en/calc.aspx 

Is there an extension or method to do this on my Magento Website in a separate page and not in the cart or product page?
