Choose to either buy once or do subscription, then pay (All on the same page)

I know this site is not built with Magento, but is there an extension that can do something like this?

So basically once you choose a flower, the next step is you choose if it's just a single purchase of bouquet OR if you want to subscribe weekly or monthly (3, 6, 12 months or longer maybe).

We are very much willing to pay.
