Order Sample Product

i'm looking for an extension that does allows the customers to order a sample for the product with a button on the product page next to the add to cart which says "order sample",

what is needed that the admin can:

1. Select what product has sample.
2. activate the sample product for Configurable products as well as simple (but most importantly configurable)
3. specify the weight of the sample product (in order to accurately calculate shipping)

i know this topic may have been discussed earlier but i couldn't find one extension that ensures the 3 requirements are met.
