personal printed card

We would like to add the option of adding a personal printed card to the order. The needs are:

1. Ask the customers during check-out whether they want this option (e.g. when they order to somebody else)

2. If yes, enable them to enter the text that will be printed on the card

3. Place the text in a template (pre-defined) and generate a high-res PDF

4. Send the PDF to our DC where it will be printed and put in the box

I am looking for an extension that will provide this functionality.
