Form to fill out for a specific product

I am running 1.9.2.3 and I am trying to add a car show registration to my store....so they can pay for their registration and fill out a form with all their info.... ie their car's make/model/year...upload a picture or pictures of their car...have a drop down menu so they can select the class they want to be in...etc.

I see I can add custom fields for the customer registration, but I don't need this info for all my products...only if they purchase this product (the car show entry)

Anyone know of how to do this? Know of a tutorial? or Know of an extension?

@Goad Probably you do not need to buy extension for these things; except you need more advance features. Magento provide Custom options features with this you can achieve your needs.  However you can also try Bundle product in case.
