adding a date selector to a form?

I have a form that when a person ask for more details about item; it asks for name, email, telephone, and some comments.

I like to add a date selector, but I am not sure how this is done. Thanks

You mentioned a form. Is that a custom form that you implemented?
If so, are you referring to a form in the front-end or back-end of Magento?
