Does Patch SUPEE-7405 include all previous patches ?
I am running v 1.8.1.0 and I have not updated or patched since 2014. Does the latest SUPEE-7405 patch include everything or do I need to install each patch individually?

It does not. You'll either need to update to the most recent version or install all patches consecutively.
