After submitting the order, redirects to cart page empty.

HI,

once submitting the order from website, order has been placed successfully, but it redirects to cart page empty. while it was happened my browser i have removed the browser cache and tried. it redirected to success page. may i know what might be the issue for this.

and also experienced unable to login , it redirects same page with out any error.

A few questions :

Which version of Magento are you using? Since how long this problem is occurring ?

Have you upgraded Magento recently?
