Cannot Manage Options (values of your attribute) in product attributes

I have a Magento 1.9.1.1 site where Manage Options under each Attribute is empty, please see screenshot. I am unable to add a new option to any attribute, or see any attribute values at all. Attributes values are visible in the front end, but the admin is not showing them.

Have cleared caches, reindexed and checked error logs and nothing is showing.

Only change major made in recent weeks was SUPEE 8788 patch which broke the product image upload routine but this was fixed with this: https://github.com/adampmoss/CreareSEO/pull/78/files#diff-0496ceab82e51dcadccb8a23074fbe17R88

Any ideas please!?

Can you see if Magento is writing something in apache log in /var/log/apache?

Also are you using any custom modules for attributes, products management?
