Import image problem Magento 1.9 not valid value

I'm getting the following error attempting to upload images.  This use to work, so not sure what I am doing wrong, is there anything wrong with my sample CSV?

Skip import row, is not valid value "" for field "type"

This is the error you get if your Image SKU's do not match and existing SKU items.

Oops! I'm new here. Didn't know solution was chosen.
