Incomplete administration panel

After changing the hosting, the login form appears incomplete without the fields for username and password or the button

Getting 500 error in shared url. Please check permission of files and folders which required to run magento.

It looks like the form is breaking https://prnt.sc/tpb4nw, have you installed any module releted admin login panel (ReCaptcha etc.)? If yes then disable it and check.

else, check the file below, has anyone made any modifications?
