I sent my admin credentials to a support person for a Magento extension because of a problem I was having. After they responded I asked my developer to see if he could fix the problem. He wrote back that he was locked out of my website admin. I am also locked out. So there were three people from the far corners of the planet trying to acess admin almost at the same time (contributing factor?) Here is the error message:
