Magento 1.9 - Product Edit Page not working, displaying incorrectly

Client is running Magento 1.9. For some reason, the edit product page is displaying incorrectly. I have searched 100+ topics and posts and can't get this fixed. I've seen a bunch of topics on the same issue but no resolution. Anyone know what could be causing this?

Please try to disable third party extension and check it.

Any solution for this one? .. If anyone can help and previously fix the same then suggestion would be great
