PLEASE HELP!! Manage Categories Page (Admin) Broken

I am trying to add more categories, and now the page looks broken. When I first click on it, it flashes an ok page, then quickly changes to some broken up code on the right side, where you enter category info.

 

PLEASE HELP!!

 

I have a screenshot, but don't know how to post the pic without uploading to server.

Found out it was a script that I'm using, that I added to index.php.. how can I add scripts and html for a 'verification to enter site' type of page otherwise?
