Random product show up in order detail - Magento 1.9.2.2

Hi,

 

If we place a order with only one product in Magento 1.9.2.2. In the order detail a random product show up. There are no rules or anything like that. Does anyone know how we can solve this?

I've never seen this before. It sounds like there's a bug with some custom code though. What extensions do you have around the ordering process or the admin area?
