Wrong customer account when logging in to site.

Having a strange issue.  When customers login, it brings up a different customer account.  I checked the account inside admin panel and everything is correct.  Did a check of the SQL database and came up with no errors.

Cleared cache and sessions.

Any ideas??

We have had this same issue off and on, I would like to have a solution to this error. 

I feel clearing the cache and session helps but this should not be the solution and should not happen. 

What is the next step in finding a solution to this cross logging in issue?
