Instructions to place a search box in a custom footer block

I'd like to place a search box in the footer of my site (the right block). 

Can any one provide me instructions or any insight for doing this? I am using M2 and Porto theme at http://metalchipsolutions.com/ and you can see my attempt in the bottom right of the footer.
