CAPTCHA not showing on blank and luma themes

I attempted to enable captcha but it won't show up. I get the Incorrect CAPTCHA message, but no captcha box. I've changed back to the luma theme, and the blank theme with same results. What should I try next?

may be some module create issue.

enable template path hint and check which phtml called.

If works then marks as solution.

Please check the Magento Log file or console when you open that particular page?
