How to create a admin form to upload file using Block ?

I have a problem while create an admin form to upload data file such as: CSV and XLS. Can anybody know how can i create an admin form that has a button to browser a file on the computer in order to select file so user can upload file into the database using Block

I just only need a template file to create a button to upload file, man. And the link you send me is upload image and it is does not have template file.  I really need serious answer
