Please verify the following things for the "Phone Number" fields:
- Login into Magento Backend
- Go to Store > Configuration
- Go to Customers > Customer Configuration
- Scroll down and check "Name and Address Options"
- Check "Show Telephone" as "Required"
- Click on the "Save Config" button
- Flush the cache:
- Click on "Cache Management" from the header notification
- Select all and click on "Submit"
