Survey and coupon

Hi,

does anyone know a good Magento extension that adds a custom questionnaire after the checkout process and gives a user that takes part some kind of coupon for his or her next purchase?

@BinchenDNP seems like you wanted to add small survey after checkout process and provide coupon to your customers if they attend. yes its possible to do. not sure about ready extension available or not. hope this help.
