Changes being erased from the website

We are working on the website and doing changes as per the requirements. 

The issue is whatever changes we do, will be lost within couple of days.

We are not able to find out why this is happening as this happened more than 4 times.

Really looking for the suggestions.

Any help would be appreciated.

Can you provide following information?

What do you mean by the changes? Are these code changes?
Which version of Magento are you using?
Have you applied all the security patches to your magento installation?

you have to check your files modification time. and database time stamp.

I have checked that as well. But could not find anything. So no one else working as well.

Can it happen because of any plugin.?

Yes, Code changes. Magento CE 1.9.2

and security patches are installed as well.

Was there a site restore done by the hosting provider? If you use version control check the current branch on production.

Did you give ftp access to some one else apart from your team in recent time?
