Table Rate Shipping Not Showing / Working...PLEASE HELP!!!

Hello,

I have Magento ver. 1.9.4.5 running and the only shipping method I have enabled is the "Table Rates" and have used the option "# of Items vs. Destination" and I have uploaded the csv file for this option. However, when I get to the checkout to test, it shows the error message for this method of shipping. I do not have any other shipping options activated. Please help! My cart currently has no shipping and in turn, people can not order due to this. Below are screenshots of my CSV file, the settings screen in the Admin, and the front end view in checkout....

can you check and upload the screenshots again?
they are unavailable. 

also, try the following : 

• Login into Magento admin 

• goto shipping method's configurations 

• switch to the website from store switcher

• export current CSV for Table Rates 

• check & update the CSV with only 1 or 2 items

That did nothing. I downloaded their CSV file originally, added a few lines of data and then uploaded it. I tried again today removing all but one line of data and still does not work. Concerning the photos I uploaded to this post, it is nice to see that even the forum for Magento has does not work like it supposed to LOL! They are simply .jpg screenshots of the CSV file, settings in admin, and the error message on checkout to show that I have everything done correctly.

Hey 

you have not added the default rates 
I have updated & attached CSV for the configuration for your table rates 

now I have added 3 rows 

1. for all country other than the USA - $25
2. anywhere in the USA $15
3. anywhere in the USA with items qty 5+ = 13.75

Note: you have also a configuration for Handling Fee = $5 so that is additional 

so for anywhere else USA, the price will be $25+$5 = $30

anywhere in the USA with items less than 5 = $20  

anywhere in the USA with items greater then equal to 5 = $13.75+$5 = $18.75

Thank you so much!!!! Shipping is working now. So, it was basically just how the cells are entered in the CSV then, correct? I do not understand the first row entries you made with the *'s for Country and such. Just happy it is working now though! Thanks again Amit!!!!
