Add custom fields to the checkout page

Hello,

I need an extension to customize the checkout page. What I mean is that I want to add some question for customer, such as “where do you know us?”, “are you satisfied with our service?”. I am using Magento 2. Please give me suggestion.

It supports different question types including 'Multiple Select' and 'Yes/No' which seem a good option for you regarding the given information.

You could also export data from custom fields in the Orders grid to .csv or .xls - along with standard Magento Order export.

It will enable you to add more fields to the checkout page and collect the needed data. The attributes info can then be added to Magento order grid and even to the HTML printing documents in the customer account.
