Hi My name is Matt and I'm Partnership and Integration manager at Mtarget SAS a french Telco company. Our job is to help our customers in setting up their SMS campaigns. We are an official SMS Aggregator routing more than a billion SMS per year for our customers. We are highly interested in offering to Magento's clients our services through "Magento Extension Builder Partner" program. 

Can any one tell me where I can find the Partner Application form ?

In that you need to click on "Technology partner" read more button - where in you will get complete details of partner program.
